During your initial periodontal appointment, our doctors will conduct a comprehensive examination. This will be a 90-minute appointment where they will examine your gums and bone, diagnose your condition, and determine a treatment plan. We will outline the fees for your individual treatment plan and the insurance coverage, if it is applicable to you.
Please assist us at the time of your initial office visit by providing the following information:
* Your dental insurance card, if applicable
* A list of medications you are currently taking
* Our completed New Patient Form, (which can be submitted here. )
* Notification if you have diabetes, hypertension, or a medical condition requiring pre-medication
Please note that all patients under 18 must be accompanied by a parent or guardian during their appointments.
